Legendary Survival Overhaul is a highly configurable mod that upgrades the survival aspect of Minecraft.
It comes with :

New coating system on armors with a sewing table to stabilize your temperature

New plants related to temperature. Find ferns in biomes to create temperature potions and insulate your armors.

Growing sun / ice ferns get you a small chance to have a golden sun / ice fern, used to craft a temperature immunity potion.
New blocks to manage your body temperature. These will cool down the space around them when activated.
Automatically detects if you're inside a house to spread the temperature effect further.

Temperature increases food consumption when you're too cold.

Hot & Cold biomes now have new outfits to survive the harsh weather. You can craft them or find them in biomes loot tables.

Depending on the type of food you'll ingest, you will increase or decrease your body temperature.

The temperature system is highly configurable. Here is a list of the things you can tweak :
Modify the player's temperature inertia
Adjust all temperature impacts from - both - the world and the player
Enable/Disable the secondary effects on hunger and thirst based on the temperature
Enable season cards to alert seasons modification (Only with Serene Seasons)
Enable & adjust the player's temperature immunity on death & at first spawn
Disable the natural regeneration of Minecraft
Disable the vanilla Freeze effect of Minecraft (in Power Snow)
Modify the temperature resistance provided by the coating mechanism
Modify the player's wetness impact on temperature
Adjust the wetness behavior and define mounts that immune to the wetness
Define the biomes where ice and sun ferns should spawn
Redefine all biome temperatures (The minecraft biome temperature is used by default), via Datapack
Define new blocks and equipments (including curios) that impact player's temperature & resistances, via Datapack

The thirst mechanic adds a new hydration bar that follows the same logic as the vanilla hunger bar (hydration + hydration saturation).

Every actions will consume a bit of hydration.
Just as temperature, this feature is highly configurable.
The hydration system interacts with the temperature system. Hot temperature will increase your thirst bar exhaustion.

To hydrate yourself, you can drink directly by right-clicking on a water block with an empty hand. Be careful though, dirty water can get you thirsty easily. (Hunger effect for thirst)

Water can be purified with furnaces. Purified water will give hydration saturation. Water can be stored in canteens. Looking up and pressing right-click when it's raining will make you drink clean rain water.

New plants & juices are added to go along with the hydration system.

Thermal Resistance Ring : Craftable by combining both rings (+3°C of both temperature resistances)
First Aid Supplies : Loot in Pillager Outposts (Your limbs regenerate passively all the time)
The thirst system is highly configurable. Here is a list of the things you can tweak :
Adjust all player actions cost in hydration
Define the hydration, saturation, effect chance & effect name for modded or vanilla blocks, via Datapack
Define the hydration, saturation, effect chance & effect name for modded or vanilla edibles, via Datapack
Adjust the Thirst Effect strength and its time accumulation
Modify the dehydration damage scaling
Adjust canteens behavior and charges
Define the biomes where the water plant should spawn
Tweak hydration values for some origins

The localized body damage is inspired by the mechanic of Fallout games that splits the limbs health and the players health.
Each damage will be attributed as accurately as possible to a specific limb.
Press H to open player limbs menu.

Broken limbs now have specific harmful effects, such as headache, vulnerability and hard falling.

Projectile Head damages are doubled. Wear a Helmet !
Limbs will need to be repaired by using healing items.

Here is a list of the effects linked to every limb damages
Head = Headache (periodic blindness)
Body = Vulnerability (All damages are 20% stronger per level)
Arms = Weakness (one arm) & Mining Fatigue (both arms)
Legs = Hard Falling (1 & 2)
Feet = Slowness (1 & 2)
The body damage system is highly configurable. Here is a list of the things you can tweak :
Adjust limbs health (dynamic or static values)
Adjust the First Aid Supplies efficiency and boosted effect
Modify the power of healing items
Add, remove or modify the effects applied by wounded limbs
Modify the headshot bonus damage
Define how much food is exhausted while while limb regenerates
Define for new damages sources the limbs it should target, via Datapack
Define healing properties onto the body limbs for modded or vanilla items, via Datapack
Define a limb resistance for each armor part, via Datapack
Health Overhaul
The Health Overhaul provides few features to both ease and improve the challenge around player health.
First of all, it provides a way to increase the player maximum health via 2 new items :
The Heart Fragments and the Heart Containers.

It also introduces the concept of Broken Heart.

Broken Hearts diminishes temporarly the player max health when a limb is fully damaged.
Be aware to not let your limbs broken, as the number of Broken Hearts inflicted is proportional to the player maximum health.
The Broken Hearts are countered by a newly introduced attribute : the Resilient Hearts. No matter how much Broken Hearts you get, the player will always keep its Resilient Hearts.
Finally, the Health Overhaul allows the player to keep the "Absorption Heart" given by the Golden Apple under the name of Shield Hearts. Those Shield Hearts are consumed when losing health, but won't disappear over time.

Even if not enabled by default, there is also the possibility to configure a loss of heart upon player's death. And the loss of heart is countered by a newly introduced attribute : the Permanent Hearts. A player won't lose the Permanent Hearts upon death.
The health overhaul is highly configurable. Here is a list of the things you can tweak :
Adjust the number of Broken Hearts inflicted due to fully damaged limbs
Adjust the maximum additional health a player can get via the Heart Containers
Adjust the maximum Shield Hearts a player can get via the Golden Apple (or any Absorption Effect)
Enable or not the conversion of temporary absorption hearts into permanent Shield Hearts
Adjust how much hearts a player lose upon death
Configure the default Resilient Hearts and the default Permanent Hearts of the player
Configure a new initial health for the players (by default, players start with 20 health = 10 hearts)
Other Features
- The debug screen menu (F3) will hide localization informations
- Compass, clocks and calendars have more infos on right-click or in a frame.
- New message when trying to use bonemeal on crops when the season won't allow it.
- New base hunger exhaustion (useful if you disable the player's natural regeneration)
